HIPAA Notice of Privacy Practices

Effective Date: April 2025

This notice describes how health information we receive, such as client diagnosis, care hours, and service preferences—may be used and disclosed, and how you can get access to this information.

Our Responsibilities

• Maintain the privacy of protected health information (PHI).
   
• Provide this notice of our legal duties and privacy practices.
   
• Abide by the terms of the current notice.
   

Permitted Uses & Disclosures

1. Treatment & Operations – We share PHI with the chosen Home Care Organization for care planning.
    
2. Payment – Minimum data necessary to invoice the HCO for our services.
    
3. Business Associates – Vendors under signed BAA (e.g., cloud VOIP, CRM).
    
4. As Required by Law – Compliance with subpoenas, public-health reporting, etc.
    

Your Rights

• Inspect and copy PHI.
   
• Request amendments.
   
• Obtain an accounting of disclosures.
   
• Request restrictions or confidential communications.
   
• File a complaint with HHS OCR if you believe your privacy rights have been violated.
